I understand that Strength and Conditioning Academy Ltd, a company incorporated in England and Wales, with registered number 13210819, whose registered office is at Kemp House, 160 City Road, London, EC1V 2NX (the Business) wishes to use Photographs and Video Footage featuring me for the purposes set out below.

For good and valuable consideration, (receipt of which I acknowledge), I confirm that:

1. I grant the business exclusive permission to use the Photographs and Video Footage:
1. in printed publications, presentations, audio presentations, video presentations, digital eLearning, in person events, and blended learning events and experiences to inform users around exercise instruction, demonstration, and safety;
2. in promotional materials and outline services and goods to current and potential clients and customers;
3. in the advertising of services and goods, specifically to advertise Strength and Conditioning Academy Ltd services and goods to potential clients and customers; and
4. on the business’ websites, specifically on strengthconditioning.academy, its sub-domains and Strength and Conditioning Academy Ltd owned websites.
(Specific Purposes)
2. I understand that my images and video footage may be edited, copied, modified, exhibited, published or distributed and waive the right to inspect or approve the finished product.
3. I waive the right to any royalties or other compensation arising or relating to the use of the Photographs and Video Footage.
4. I consent to the Business:
1. storing copies of the Photographs and video footage for the Specified Purposes or to it storing my contact details on its database in case it needs to contact me.
5. I have the right to:
1. withdraw my consent to the Business using and storing copies of the Photographs and Video Footage for specified purposes at any time;
2. withdraw my consent to the Business storing my contact details on its database;
3. access the Photographs and Video Footage and any information the Business holds on me at any time; and
4. request the Photographs and Video Footage and any information the Business holds about the Photographs and Video Footage, and any information the Business holds about me to be removed from any materials, future materials, and internal database by contacting the Business at the email address: Admin@StrengthConditioning.Academy.
6. The Agreement and any dispute or claim arising out of or in connection with it or its subject matter or formation (including non-contractual disputes or claims) shall be governed by and construed in accordance with the law of England and Wales. Each party irrevocably agrees that the courts of England and Wales shall have exclusive jurisdiction to settle any dispute or claim arising out of or in connection with this agreement or its subject matter or formation (including non-contractual disputes or claims).
